We have read the article “Computed tomography whirl sign: An image-based indication for surgery.” with great interest [1]. Hobeika et al. reported that they had a 77-year-old female patient with a history of aortic bypass surgery with suspected small bowel obstruction. The patient was evaluated with thoracoabdominal computerized tomography (CT). Computerized tomography scan revealed whirl sign which urged the surgeons to operate. Small bowel volvulus was detected intraoperatively, thus adhesiolysis was performed. In addition to whirl sign in small bowel volvulus, we would like to contribute to this brief report with whirl sign in sigmoid volvulus. The rotation of the sigmoid colon around its mesentery leads to sigmoid volvulus. Rotation of the intestines around their mesentery with mesenteric vessels causes the radiological whirl sign. A ninety-year-old patient complaining of constipation, abdominal pain and distension was admitted to the intensive care unit. The abdominal CT scan revealed both coffee bean sign/omega sign and whirl sign confirming sigmoid volvulus (Fig. 1). Endoscopic detorsion could not be achieved. Therefore, the patient underwent Hartmann procedure, and he was discharged home on postoperative day six without any complications. However, whirl sign is not pathognomonic for sigmoid volvulus [2], [3]. Whirl sign may also be encountered in ovarian torsion, cecal volvulus, closed loop intestinal obstruction and omental torsion. Nevertheless, physical examination findings combined with radiological whirl sign help the surgeon to reach a preliminary diagnosis prior to surgery in patients with sigmoid volvulus.
