    ChatGPT and pediatric advanced life support: A performance evaluation

    The development of artificial intelligence (AI) tools, such as large language models (LLMs), holds significant promise for enhancing patient care and medical education. ChatGPT (Chat Generative Pre-trained Transformer), an LLM developed by OpenAI utilizing the GPT-4 architecture, currently demonstrates the highest level of medical domain knowledge among its peers.1 While ChatGPT’s performance has been assessed in various medical examinations,2,3 its capabilities in pediatric resuscitation and advanced life support remain unexplored. This study aimed to evaluate the clinical reasoning ability of ChatGPT by testing its performance on the American Heart Association (AHA) Pediatric Advanced Life Support (PALS) exam.

    Traumatic Head Injuries in Infants

Thanks to the authors and editorial board for this informative and interesting article. However, we would like to point out a few points about infant trauma that may contribute to the discussion of the article.

    Ocular trauma in the pediatric age group: a systematic review

    Ocular traumas are the leading cause of acquired unilateral blindness in the pediatric age group. The aim of this study is to determine the incidence of pediatric ocular injuries, the etiological factors that cause them, and to make some preventive recommendations. Methods: This was a retrospective observational study of pediatric ocular trauma (age < 18 years) presented to the pediatric emergency unit of our tertiary university hospital between January 2016 and December 2020. Demographic data such as age, gender, season, duration of admission, trauma etiology, and type of injury were recorded. Patients with missing medical records were excluded from the study. Results: Two hundred two patients aged a mean of 107.4 +/- 40.7 months including 87 females (43.07%) and 115 males (56.93%) were included in the study. Most of the patients (n = 135, 66.83%) were in the 7-11 years of age group. The vast majority of the study population lived in the urban area (n = 119, 58.91%). Most injuries were seen in the summer (n = 108, 53.47%). 59.41% of all patients had closed globe injuries. Hyphema (32.5%) and endophthalmitis (26.6%) were the most common in closed globe injuries. Of the open globe injuries, 39 were penetrating (47.5%), 12 were perforated (14.6%), and 23 were intraocular foreign body injuries (28%). It was determined that the places where the injury occurred were mostly in situations where family control was reduced, such as weddings, holidays, and playgrounds (80.69%). The most common causes of injury were toy guns (35.15%), stones (28.71%), and fireworks (16.34%). Conclusion: This analysis is the first study to provide data on childhood ocular trauma in Aksaray. Most of the childhood ocular injuries can be prevented with simple precautions. For this purpose, effective and preventive strategies should be developed such as raising public awareness, providing parental control, and early intervention.

    Konjenital Vitreus patolojilerinde Vitrektomi

    Konjenital vitreus hastalıkları, vitreusun embriyolojik gelişim kusurlarının sonucu oluşmakta ve bu hastalıklar, bozulmuş vitreus yapısını içeren geniş bir yelpazeye sahiptir. Aynı zamanda bu hastalıklar, çocukluk dönemi görülen vitreus ve retinal patolojilerde ilk akla gelen hastalıklardır. Birçoğunda pars plana vitrektominin, tedavide önemli bir rolü vardır. Hastalık patofizyolojisi ve hastaların yaşı nedeniyle, rutin uygulanan pars plana vitrektomide yöntemlerinde farklılıklar mevcuttur. Bu derlemede konjenital vitreus patolojileri ve bu patolojilerin tedavisinde uygulanan vitrektomi yöntemi tartışılacaktır.

    Dear Editor Omalizumab is a humanized monoclonal antibody against human immunoglobulin E that is mainly indicated in asthma, allergic rhinitis, chronic spontaneous urticaria and is being increasingly used for other dermatological diseases, including atopic dermatitis, mastocytosis, hyper-IgE syndrome, and bullous pemphigoid.1,2 Rare cutaneous side effects, such as itching, skin rash, urticaria, and photosensitivity, have been reported to be associated with omalizumab use.

    Bonsai-induced kounis syndrome in a young male patient

    To our knowledge, this patient is the first bonsai-induced Kounis syndrome case in literature. Kounis syndrome, in other words allergic MI, has two types depending on the pathophysiology, or the presence of coronary artery disease. In type I, patients exhibit coronary vasospasms induced by allergic mediators such as histamine, thromboxane, and leukotrienes without the presence of atherosclerosis risk factors or coronary artery disease. In type 2, ACS develops due to coronary vasospasms, plaque erosion, or plaque rupture induced by these mediators in patients with atherosclerotic coronary artery disease. Recently, the fact that there are eosinophil and mast cells in the thrombus material excised from some patients in whom stent thrombosis developed after stent implantation with drug release makes us consider hypersensitivity reactions in these patients. This situation is accepted as the type III variant of Kounis Syndrome (4). With these findings, our case is in accordance with the type I variant of Kounis syndrome

    Cardiac manifestations in Behcet's disease

    Behcet's disease (BD) is a chronic inflammatory disorder, with vasculitis underlying the pathophysiology of its multisystemic effects. Venous pathology and thrombotic complications are hallmarks of BD. However, it has been increasingly recognised that cardiac involvement and arterial complications are also important aspects of the course of the disease. Cardiac lesions include pericarditis, endocarditis, intracardiac thrombosis, myocardial infarction, endomyocardial fibrosis, and myocardial aneurysm. Treatment of cardiovascular involvement in BD is largely empirical, and is aimed towards suppressing the vasculitis. The most challenging aspect seems to be the treatment of arterial aneurysms and thromboses due to the associated risk of bleeding. When the prognosis of cardiac involvement in BD is not good, recovery can be achieved through oral anticoagulation, immunosuppressive therapy, and colchicine use. In this review, we summarise the cardiovascular involvement, different manifestations, and treatment of BD.
