    Unexpected outcome in a patient presenting with syncope: splenic laceration

    One of the most frequently injured organs in blunt trauma is the spleen. Typical symptoms of spleen injury are left-sided abdominal pain, peritonitis, and hypovolemic shock. Kehr sign can be seen in approximately 20% of cases. It is a rare case of a splenic injury where the abdominal examination is comfortable and the patient does not complain of abdominal pain. A 68-year-old male patient applied to the emergency department with the complaint of fainting after feeling dizzy while leaving the mosque. At the time of admission, blood pressure was measured as 126/68 mmHg, pulse 86/min, SpO2 97%, temperature 36.4 °C, fingertip blood sugar 98 mg/dL, and he did not have any active complaints. In the physical examination of the patient; His GCS was 15, he was oriented and cooperative, and his neurological examination was normal. There was no guarding, rebounding or tenderness on abdominal examination. His rectal examination revealed colic and stool contamination. There were no acute pathological findings on the patient's brain CT. The patient, whose vitals remained stable during follow-up, declared that he felt dizzy again when he was lifted off the stretcher during the control examination. The blood pressure of the patient, whose vitals were checked, was measured as 70/40 mmHg. The patient stated that he had a new onset of abdominal pain. When the anamnesis was detailed, he stated that he hit his left side on the pavement while falling, but he did not mention it at the first anamnesis because he had no pain. Splenectomy was performed on the patient, whose spleen injury was detected on abdominal imaging and general surgery was consulted. With this case report, we aimed to remind once again the importance of follow-up examinations, and that in addition to non-traumatic causes, a detailed anamnesis should be kept and the trauma history should be questioned in detail in the patient presenting with syncope.

    Graves' thyrotoxicosis case presenting with COVID-19

    Graves' disease is an autoimmune multifactorial thyroid disease caused by environmental factors in genetically susceptible patients. Viral infections are thought to play a role in the pathophysiology of this disease. Several potential mechanisms have been proposed for the pathophysiology of Graves' disease, including antigen exposure, cytokine release and inflammatory response. A review of the literature shows that subacute thyroiditis developing after viral infection has been reported as Graves' disease. It is a potential risk factor for Graves' disease and should be considered in the differential diagnosis in the etiology. For this purpose, we present a case compatible with Graves' disease that developed after COVID-19 viral infection, which will contribute to the literature.

    In order to ensure that eating behaviors are healthy, more studies are carried out in order to increase eating awareness. Eating awareness can be used together with other known healthy eating behavior techniques. It is thought that eating awareness may be important in the treatment or prevention of obesity and eating disorders. In this review, literature information about the concept of eating awareness and its effect on obesity and eating disorders will be presented.

    Comparison of conjunctival flora of Turkish and Syrian individuals

    Aim: The refugee problem is a problem that has been going on all over the world for many years and has reached increasingly larger dimensions. Turkey is one of the leading countries hosting Syrian refugees. In this study, we aimed to evaluate the conjunctival flora of Syrian refugees and Turkish individuals admitted to our clinic and to compare the results. Materials and Methods: A total of 96 patients, 55 Turkish and 41 Syrian, were included in the study. A swab was taken from the conjunctiva of the lower eyelid of the patients with a sterile swab. The samples were cultured on Eosin Methylene Blue (EMB) agar, Sabouraud dextrose agar, chocolate agar and 5% sheep blood agar media in the microbiology laboratory. Results: Conjunctival culture positivity was 53.7% in Syrian refugees and 52.7% in Turkish individuals (p=0.92). Coagulase Negative Staphylococcus (CNS) was the most frequently isolated microorganism in Turks (32.7%) and Syrian refugees (46.3%) (p=0.19). CNS was isolated in 18 (32.7%) Turks, Streptococcus spp. was isolated in 10 (18.2%), Staphylococcus aureus in 7 (12.7), Corynebacterium spp. in 6 (10.9%), Bacillus spp. in 7 (12.7%), Micrococcus spp. in 1 (1.8%), Neisseria spp. in 1 (1.8%). In conjunctival cultures of Syrian refugees, CNS was isolated in 19 (46.3%), Corynebacterium spp. in 7 (17.1%), Streptococcus spp. in 3 (7.3%), Staphylococcus aureus in 2 (4.9%), Bacillus spp. in 5 (12.2%), Micrococcus spp. in 1 (1%). Conclusion: Conjunctival growth was similar in Turks and Syrian refugees and CNS was the most frequently isolated bacterium. The bacterial species isolated were similar in the two groups and there was no statistically significant difference in the frequency of these bacteria.

    Comparison of 2019 and 2020 outpatient data of a Mental Health Hospital: the impact of the COVID-19 outbreak

    Aim: In this study, it was aimed to examine and compare the sociodemographic and clinical variables of the subjects admitted to the outpatient psychiatry clinic of a mental health and diseases hospital (MHDH). Material and Method: All subjects who admitted to the outpatient psychiatric outpatient clinics of XXX MHDH between 01.01.2019-31.12.2019 and 01.01.2020-31.12.2020 were included in the study. Sociodemographic and clinical data of the subjects, such as age, gender, and diagnosis, were recorded. Results: The number of subjects admitted between 01.01.2019-31.12.2020 was 108,367 (71,280 in 2019; 37,087 in 2020). The total mean age was 44.79±17.62 years (45.18±17.78 years in 2019, 44.03±17.28 years in 2020). In total, there was a significant difference between the ages of male and female subjects (p<0.001) and 75.1% of the subjects resided in XXX. In total, 85.9% of the subjects were admitted due to examination, 4.6% due to a single physician report, 5.1% due to medical board report, and 4.4% due to probation. Overall, there were differences between genders in terms of admission reason, diagnosis, hospitalization after outpatient examination, and health insurance (p<0.001). In total, the most common diagnoses were anxiety disorders spectrum (21.6%), schizophrenia spectrum disorders (19.4%), depression spectrum disorders (15.6%), and bipolar spectrum disorders (12.5%). Forensic health insurance, residence, and diagnosis in hospitalized subjects were significantly different from those who were not hospitalized (p<0.001). Ninety-four percent of the forensic admissions were male, 55.9% admitted for probation, 58.5% were diagnosed with alcohol and substance use disorder, 87.7% of those who admitted for the medical board report were diagnosed with a general psychiatric examination. In total, the rates of bipolar spectrum disorder and schizophrenia spectrum disorder were higher in those residing outside XXX, while anxiety disorder spectrum and depression spectrum disorder were higher in those residing in XXX (p<0.001). Conclusion: The COVID-19 outbreak was associated with a significant decrease in the number of subjects admitted to the outpatient psychiatric clinic of an MHDH. In addition, there were various differences associated with COVID-19 in terms of health insurance, admission reason, diagnosis, and hospitalization between the subjects admitted in 2019 and 2020.

    Antimicrobial susceptibility patterns and extended-spectrum ?-Lactamase production by enterobacterales in a tertiary hospital

    Objective: This research aims to characterize extended-spectrum beta-lactamase (ESBL) producers and assess the antimicrobial susceptibility patterns of Enterobacteriaceae isolated from clinical care patients in the Aksaray province, Turkey, spanning from January 2017 to December 2021. Material and Methods: Clinical strains will be microbiologically isolated and identified. Antibiotic resistance profiles of Enterobacteriaceae isolates will be determined, and the presence of the blaCTX-M gene will be investigated using molecular methods. Results: Initially, 1752 clinical strains were isolated and identified using VITEK® 2 with an automated ID and susceptibility system. Subsequently, identified and suspected isolates were subjected to PCR for blaCTX-M gene detection. Klebsiella pneumoniae (43.95%) was more common than other species, including Escherichia coli (45.89%). Antimicrobial susceptibility revealed resistance percentages of 86.62% to ampicillin, 76.33% to amoxicillin/clavulanic acid, 64.52% to cefuroxime, 59.14% to trimethoprim/sulfamethoxazole, 59.99% to ceftriaxone, 58.35% to ceftazidime, and 54.39% to ciprofloxacin. Fosfomycin, nitrofurantoin, imipenem, and amikacin exhibited sensitivity in 45.49% of urine isolates. ESBL positivity was observed in 47.63% of E. coli and 61.16% of K. pneumoniae isolates. Conclusions: The prevalence of ESBL-producing E. coli was relatively high. Antimicrobial resistance is a complex issue influenced by widespread antibiotic usage. Insights from trend research can contribute to a broader assessment of antibiotic resistance profiles in Turkish provinces.
